  1. Are you currently a member of any church?
  2. Do you attend church regularly?
  3.
  4. Have you made a personal commitment to and do you have a present relationship with Jesus Christ?
  5. Do you agree with or have respect for the following summary of the Articles of Religion of The Wesleyan Church?

I believe in God the Father, the Son and the Holy Spirit.
I believe that Jesus Christ the Son suffered in our place on the cross, that he died but rose again, that He now sits at the Father's right hand until He returns to judge all men at the last day.
I believe in the Holy Scriptures as the inspired and inerrant Word of God.
I believe that by the grace of God every person has the ability and responsibility to choose between right and wrong and that those who repent of their sin and believe in the Lord Jesus Christ are justified by faith.
I believe that God not only counts the believer as righteous but that He makes him righteous, freeing him of sin's dominion at conversion, purifying his heart by faith and perfecting him in love at entire sanctification, and providing for his growth in grace at every state of his spiritual life, enabling him through the presence and power of the Holy Spirit to live a victorious life.

  6. The Wesleyan Church Corporation requires that all employees abide by a Christian and moral code of conduct. Do you agree with and will you conduct your life in accord with the following statements which reflect the Church's position regarding moral behavior?
    1. SUBSTANCE ABUSE. The human body, as the temple of the Holy Spirit, should be maintained in health, not abused by overeating, or by failure to get sufficient exercise or rest. The production, sale, purchase and use of alcoholic beverages, tobacco, narcotics and other harmful drugs should be avoided entirely.
    2. MARRIAGE AND SEXUALITY. Human sexuality is to be expressed only within the monogamous, heterosexual framework of marriage. Marriage is a lifelong relationship and a covenant union made in the sight of God and should take priority over all other human relationships. Homosexual practice is immoral and sinful, but redemption and restoration are possible through God's grace.
    3. SANCTITY OF LIFE. There are no justifiable grounds to deliberately end the life of an individual through medical or any other means. In principle, infanticide and euthanasia are wrong. Other than in rare pregnancies where grave medical conditions threaten the life of the mother, the use of induced abortion is not acceptable.
