Participant Investment Election Enrollment form used by participants to make an initial investment election. This form along with the Beneficiary form is to be completed and returned to The Wesleyan Pension Fund office before contributions are remitted to an account.
Designation of Beneficiary Beneficiary form is to be used by a participant to designate a primary and/or contingent beneficiary. This should be completed and returned with the Participant Investment Election form.

Salary Reduction Agreement An agreement that the participant and the employer complete to permit participant to make salary reduction contributions to the Pension Plan.

Salary Reduction Amendment Agreement that the participant and the employer complete to permit the participant to amend their salary reduction agreement with the Pension Plan.

Transfer Statement and Agreement Form that a participant must submit in order to transfer assets into the Wesleyan Pension Fund.

Partial Withdrawal This form is used for partial distributions from the Pension Plan for those participants after age 62 or older
Lump Sum Distribution This form is to be used for a lump sum withdrawal from the Pension Plan. NOTE: No payment of pre-retirement termination benefits will be made if the participant remains ordained, commissioned or a licensed minister with TWC until age 62. There is a minimum six-month waiting period from date of separation of employment, ordination, commission or licenses. Please refer to the Summary Plan Description, page 15 for details.

W-4P Federal Income Tax Withholding IRS form for all retirees who file an application for benefits (extended payment or partial withdrawal) providing information on Federal tax withholding on annuity and installment payments.

Remittance These forms are used for the remittance of pension contributions. Contact the WPF office at #800-595-4131 for a supply of the forms and self-addressed envelopes (no cost). The remittance forms are to be mailed to our lockbox located at: Wesleyan Pension Fund, P.O. Box 632953, Cincinnati, Ohio 45263-2953.
